Visiting Doctor and Nurse Practitioner Services

Our medical providers bring professional evaluation and follow-up care to patients who have difficulty traveling to a medical office. Depending on the patient’s needs, services may be provided at home, through telehealth, or in our clinic.

Advanced Wound Care at Home or in Our Outpatient Clinic

Our wound-care team evaluates and manages complex and non-healing wounds. Each treatment plan is developed according to the patient’s wound type, medical history, circulation, mobility, and overall health.

Our vascular services help identify and manage circulation problems that may contribute to leg pain, swelling, skin changes, and delayed wound healing. Patients may receive care in the clinic or be referred for appropriate vascular testing and treatment.

Transitional Care Management After Hospital Discharge

NYC HouseCall Medical supports patients during the critical transition from a hospital or rehabilitation facility back to the community. Our goal is to reduce gaps in care, address concerns early, and help patients recover safely at home.

Transitional Care Management may include:

• • Contact with the patient or caregiver within 48 hours after discharge
